DOT/CDL Physicals

Dr. Gibson has been doing DOT physicals for since 2008 here at Marsing Chiropractic, and has been working out of his office since 2007.  We try to make your DOT/CDL exam a relaxing and comfortable experience by making you the focus of our efforts during the exam. 

Our office has added an Electronic DOT/CDL form program that shortens the amount of time and paperwork needed to be filled out for this exam.  This will also help with follow up exams, because you will only need to update your information and put your signature at the bottom of the form.  We understand that this DOT/CDL process can be difficult at times, and we seek to make this process as easy as possible. 

For appointment times it is recommended that you make an appointment to be seen, because the whole time from start to finish is about 45 min. to 1 hour.  Appointments days and times are as follow Monday, Tuesday, Friday are 9 to 5, Thursday 9-6:30 and Saturday is by appointment only. For those who are licensed in the state of Idaho as a CMV operator for you convenience there is a state DMV office next to our office with hours from 8:30 to 3:30.

Items you may need for your DOT Physicals

Heart Problems - Heart Attack, Heart Stents, Pacemakers, Bypass, Transplant, valve replacement... etc.

*You will need to bring a clearance letter from your Cardio Doctor that your treatment is Safe, Stable and Effective to operate a CMV or school bus.

Diabetes - Are you using Insulin?

*You will need to bring your most recent Hemoglobin A1c lab test (w/in the last 3-mo).

*If you are using Insulin you will need to have your prescribing Doctor fill out form MCSA-5870 and Hemoglobin A1c lab test (w/in the last 3-mo). You can get this form from Dr. Gibson, or you can down load it from my site. 

Anxiety, Depression or other Mental health issues that require a prescription: 

*You will need a letter from your prescribing Doctor that you are on this medication and that your treatment for this issue is Safe, Stable and Effective for you to operate a CMV or school bus.

CPAP use - to help you breath when you sleep:

*You will need to bring a 90-DAY usage report that shows usage of >/= 70%. 

If you answered yes to any of the above you will need to bring the required documentation. If you cannot supply any of this documentation at the time of your DOT Physical you will be placed in a pending category for 45-days or until your current Medical Certificate expires, whichever is less.
